ServicesOur nurses, physical therapists, occupational therapists, and licensed nursing assistants provide home care for patients recovering from hospitalization or illness. Enhanced Specialty Program for Home Care Clients with a Diagnosis of Congestive Heart Failure (CHF) This program involves the use of HomMed telehealth monitoring , a ZOEŽ machine, client education tools, and assessments performed by a registered nurse trained in cardiac care. The goal of the program is to improve client outcomes and quality of life. NANA sponsors the Senior Companion Program and the Newfound Area MS Self Help Group. Free Blood Pressure, Foot Care and Well Child Clinics and low-cost Flu Shot Clinics are held on a scheduled basis.
